- Submit the Parents’ Financial Statement to SSS online. CFS school code is 2014. We encourage you to receive a family report. Family report code is 9000.
- Submit a copy of the final pay stub(s) for 2010, showing year-to-date earnings and pre-tax benefits.
- SSS Business/Farm Statement (if applicable) if you are self-employed, own a business or a farm.

- Copy of all 2010 W-2 earning statement(s) for working adults in the household.
- New applicants to aid only - copy of a signed, completed 2009 federal tax return, including all schedules.
- Signed , but not dated, form 4506-T
- We appreciate a copy of completed, signed, 2010 federal tax return, including all schedules by February 4 to SSS. The final tax document, however, is not due to SSS until April 15. Please note that awards granted are contingent upon receiving the final copy by April 15, and CFS reserves the right to recalculate an award if information received or missing from the final 2010 tax return warrants another review.

- Tuition assistance decisions are mailed to new students meeting our deadlines. Grant offers must be signed and returned to CFS by April 11, 2011.

| April 15, 2011 |
- Final, complete, signed 2010 federal tax return deadline. If you have filed for an extension, please submit IRS form 4868.
- The tuition assistance committee reserves the right to recalculate a financial aid award if information received or missing from the final 2010 tax return warrants another review.
